At jakegurlandmd.com, we offer a range of medical services including general check-ups, vaccinations, specialized treatments for chronic illnesses, covid-19, flu and strep testing, nebulizer treatments, A1c's in office and allergy testing. 


We are open from Monday to Thursday 8am to 4pm and Friday 8am to 12pm. Closed Saturday and Sunday. We do have our service voicemail on over the weekend. Please feel free to leave a voicemail for all NON Emergencies and our office staff will get back to you the next business day. If this is a medical emergency please call 911 or head to the nearest hospital.

You can obtain your medical records from your patient portal, by logging into healow.

  • If you wish to have a printed copy of your chart, a request will need to be made in writing and pricing will be $100 per page up to $100.
  •  If you wish to have the office forward your records to another doctor or a specialist, please sign a release of records and provide us with the providers information. The cost for this process is free.

Please note our most current office policies below. Should you have any questions please feel free to reach out to our office manager. Thank you. 

  • Prescription History Authorization: I authorize Jake Gurland, MD to view my external prescription history. I understand that prescription history from multiple other unaffiliated medical providers, insurance companies, and pharmacy benefit managers may be viewable by my providers and staff here, and it may include prescriptions back in time for several years.
  • All controlled medications and GLP-1 will require an office visit every 30 days, no exceptions. Please set up all visits one week prior to your next refill to avoid any delays.
  • For Cancellations - Please cancel within 24 hours of your appointment time, if not a $25.00 charge will be placed on the account.
  • Co-pays are to be paid prior to your telehealth visit. A phone call and text (with link to pay) will be made to you prior to your appointment. If you are unreachable, your appointment will be considered a No Call No Show after 15 minutes and you will be billed the $25 charge.
  • When running late for your appointment time, please reach out to the office and kindly let us know, as we allow a 15-minute grace for all appointments. After 15 minutes you will be considered a No Call No Show and be charged the $25.00 fee.
  •  Annual Wellness visit/ Routine physical exam: A preventive visit does not include treatment of an acute illness or management of a chronic condition. If these problems are addressed during a wellness visit, a separate problem focused office visit may be billed.
